Eddie Yagin pulled out of his UFC on FOX 5 bout against Dennis Siver after being admitted to the hospital and Nam Phan has agreed to face Siver on December 8th in Seattle. 
UFC officials announced the change on Wednesday and the fight will take place on the preliminary card of UFC on FOX 5.
Phan (18-10) is a TUF alum and has gone 2-3 in his five UFC fights. He has won two of his last three fights with the victories coming against Leonard Garcia and Cole Miller. All five of his UFC fights have gone to the decision and has won two fight of the night bonuses during his UFC career.
Siver (20-8) is in his second stint in the UFC and has gone 8-2 since returning to the UFC. He was scheduled to meet Yagin at UFC 151, but the fight was moved to UFC on FOX 5 after the 151 card was canceled. Four of his last five wins have come by decision and holds UFC victories over George Sotiropoulos, Matt Wiman and Diego Nunes.
UFC on FOX 5 takes place from KeyArena in Seattle and will be headlined by Benson Henderson defending the UFC lightweight title against Nate Diaz.
